2011 Health Examination (Medical Checkup)

For the new international students

Dear all new international students;

We will have the Health Examination (Medical Checkup) for all new international students on October 14th, Friday. You are recommended to take the examination to maintain your health condition in a different environment.

Date: October 14th, Friday, 2011

Time: 9:30 a.m.-11:00 a.m.

(Female students (Only for female students whose Student No. is Y11-SXXX or Y11-PXXX))

11:00 a.m-12:30 p.m. (All male students)

13:30 p.m.-15:00 p.m. (Other female students)

Place: Student Health & Counseling Service Center on campus, 1^st floor

　　　　(Building #22 on Campus map)

Examinations:

Chest X-ray / Medical Examination by a doctor / Eyesight, Height, Weight

l Please bring your STUDENT ID CARD　and medical interview sheet with you.

l It is recommended to be on white T-shirt and no accessories when you take the examination.

l Be sure to take all kinds of examinations so that you will receive the examination result. The medical certificate will be issued in about 3 weeks after the examination, bring your student ID card to get it at International Division.

(対象2011年10月入学生)

２０１１年１０月入学の留学生を対象に下記の日時に健康診断を実施いたします。風土や気候、食生活習慣の異なる国に住みはじめると体調を崩しやすくなります。病気の予防や健康管理に役立てるように、留学生の皆さんも必ず受診してください。

日時：２０１１年１０月１４日（金）

　　　9:30 a.m〜11:00 a.m （学生番号がY11-S,Y11-Pから始まる女性）

11：00 a.m.〜12:30 p.m.（男性留学生全員）

　　　13:30 p.m.〜15:00 p.m. (その他女性)

場所：保健管理センター１階（建物番号２２）

健康診断項目：�@胸部レントゲン間接撮影　

�A内科検診

�B視力・身長・体重測定

※ 学生証（身分証明書）と健康診断受診票を持参してください。

※ レントゲン撮影のため、Tシャツ着用を勧めます。アクセサリーなども外してください。

※ すべての項目について検査を受けるようにして下さい。検査結果は、健康診断当日から約３週間後にお渡しします。本人が学生証（身分証明書）を持参し、国際課でお受け取りください。
